☐ I understand my coach will not make major decisions for me.
☐ I am willing to participate in weekly private coaching for approximately 12 weeks.
☐ I am willing to practice what I learn between sessions.
☐ I understand the Founding Five investment is $1,650 paid in full or three monthly payments of $600.
